PCCAG have provided support to the PCTs to allow information required for the LDP to be collected from GP Practices. This has so far included information around Smoking & Obesity. This was collected at a county level and was used to compile LDP returns. It is also being used by the Smoking Cessation service to complete a countywide picture on smoking status.
